- Last date of availability and submission of application forms with late fee Rs.500/-: 21st Jan 2014 (10.00 a.m. to 06.00 p.m.)
- The application form downloaded after 18th Jan 2014 is to be submitted in person and/or post should reach to this office on or before 21st Jan 2014 along with demand draft

A candidate desirous of appearing for All India Post Graduate Medical and Dental Entrance Test of this University will be required to submit application in the prescribed form along with the attested copies of the documents in support of the applicationThe application form and brochure can be obtained from university office or by downloading till 18th Jan 2014 from university website www.sumandeepuniversity.co.in, and is to be submitted along with demand draft of Rs.3500/- (form fee plus test fee)
The application form downloaded after 18th Jan 2014 is to be submitted along with demand draft ofRs.4000/- (form fee plus test fee plus late fee)
Prospectus and application form Rs.1000/-; entrance test fees Rs.2500/-
The fee is payable by a demand draft, drawn in favor of Sumandeep Vidyapeeth of nationalized/schedule bank, payable at Vadodara
The candidate should write his/her name, address and mobile number on the reverse side of the demand draft
The last date of receipt of completed application form along with requisite documents and entrance test fee at the office of Registrar, Sumandeep Vidyapeeth Piparia on 18th Jan 2014, upto 02.00 p.m. and with late fee of Rs.500/- on 21st Jan 2014, 06.00 p.m.
